On-site audits (Level 4)

How an L4 audit is requested, scheduled and scored, what the auditor checks, and how corrective actions work.

A Level 4 on-site audit confirms that what happens on your jobs matches your paperwork. It earns the gold Audited badge, valid for 24 months.

Who can request it

  • Contractors can request an audit of their own company once Levels 1-3 are verified.
  • Companies can request an audit of a contractor in their network. The company pays, or uses an included audit credit on Enterprise.

Start from Request an on-site audit at /audits/request.

The steps

  1. Request and payment. Pay by card, or with an Enterprise audit credit.
  2. Auditor match. We match an independent auditor who covers your state and trade and has capacity that month.
  3. Scheduling. The auditor proposes several times; the contractor picks one.
  4. Site visit. The auditor works through a standard checklist on a live job, with photos geotagged on site.
  5. QA review. Our team reviews the report for completeness and consistency.
  6. Comment window. The contractor sees the draft first and has 5 days to comment.
  7. Final report. The report is published to the profile with section scores. The badge is valid for 24 months.
  8. Corrective actions. Each failed item becomes a corrective action due in 30 days, tracked to closure.

What the auditor checks

Training recordsOSHA cards, lift cards, manufacturer and trade certifications matching the crew on site

Each section is scored 0 to 100 from the items that apply. The overall score feeds the Safety sub-score of the Trust Score.

Corrective actions

Respond to each one from the audit page with what you changed and upload evidence (photos, training records, a revised program). Our team closes it or asks for more. Open items stay visible to the company that requested the audit.

Paying for an audit never changes the result, or any score, ranking or badge, beyond what the audit itself finds.
